Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurer issuing policies shall apply to the Chief Controlling Revenue Authority in Form D for a duplicate, accompanied by a fee of fifty rupees, and the Chief Controlling Revenue Authority shall issue a duplicate certificate marked as such within one hundred and twenty days of the application.
