Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the certification of aircraft maintenance is carried on, the particulars of every navigation aid held by the applicant and such other particulars as are specified in Form CA-2, and shall be signed by the applicant or his authorised agent.
