Surrender of certificate.—Every self-employed person who ceases to carry on the carrying on of a self-employed occupation shall, within ten days of such cessation, surrender the certificate of registration to the Regional Board in person or by registered post, and shall inform it in Form 5 of the manner in which every claim held by him has been disposed of.
