Surrender of certificate.—Every dependant of a deceased worker who ceases to carry on the claiming of dependants' benefit shall, within fifteen days of such cessation, surrender the certificate of registration to the Employees' Insurance Court in person or by registered post, and shall inform it in Form 12 of the manner in which every identity card held by him has been disposed of.
