Application for registration.—Every dependant of a deceased worker carrying on the claiming of dependants' benefit shall make an application for registration to the Employees' Insurance Court in Form 12, within twenty-one days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every declaration form held by him on the date of the application, and shall keep a copy of the application and of the statement for forty-two days from that date.
