Rule 22, Madhya Pradesh Mines and Minerals (Development and Regulation) Act, 1957 (Manner of Application) Rules, 2012 [SCK-YYE]

Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the consumer shall apply to the Appropriate Commission in Form II for a duplicate, accompanied by a fee of fifty thousand rupees, and the Appropriate Commission shall issue a duplicate certificate marked as such within one hundred and twenty days of the application.
