Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the service provider shall apply to the Central Government in Form C for a duplicate, accompanied by a fee of ten thousand rupees, and the Central Government shall issue a duplicate certificate marked as such within ninety days of the application.
