Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a residential facility shall apply to the District Level Committee in Form VII for a duplicate, accompanied by a fee of fifty rupees, and the District Level Committee shall issue a duplicate certificate marked as such within thirty days of the application.
