Regulation 21, CERC (Forms of Distribution licensees) Regulations, 2011 [SCJ-10C]

Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the distribution licensee shall apply to the Grievance Redressal Forum in Form IV for a duplicate, accompanied by a fee of two thousand rupees, and the Grievance Redressal Forum shall issue a duplicate certificate marked as such within ninety days of the application.
