Application for registration.—Every dependant of a deceased worker carrying on the claiming of dependants' benefit shall make an application for registration to the Welfare Board in Form 12, within sixty days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every medical certificate held by him on the date of the application, and shall keep a copy of the application and of the statement for thirty days from that date.
