Surrender of certificate.—Every dependant of a deceased worker who ceases to carry on the claiming of dependants' benefit shall, within thirty days of such cessation, surrender the certificate of registration to the Welfare Board in person or by registered post, and shall inform it in Form 3 of the manner in which every contribution held by him has been disposed of.
