Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the dependant of a deceased worker shall apply to the Welfare Board in Form 9 for a duplicate, accompanied by a fee of ten rupees, and the Welfare Board shall issue a duplicate certificate marked as such within ninety days of the application.
