Intimation of change.—Every unorganised worker shall intimate to the Medical Benefit Council, in Form 5 and within ninety days of the change, any change in the particulars furnished in the application for registration, and the Medical Benefit Council shall amend the certificate accordingly on payment of a fee of two hundred rupees.
