Application for registration.—Every unorganised worker carrying on work in the unorganised sector shall make an application for registration to the Medical Benefit Council in Form 12, within sixty days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every accident report held by him on the date of the application, and shall keep a copy of the application and of the statement for ten days from that date.
