Surrender of certificate.—Every establishment employing persons with disabilities who ceases to carry on the employment of persons with disabilities shall, within fifteen days of such cessation, surrender the certificate of registration to the Government of West Bengal in person or by registered post, and shall inform it in Form IV of the manner in which every assistive device held by him has been disposed of.
