Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a hospital shall apply to the State Authority in Form F for a duplicate, accompanied by a fee of one thousand rupees, and the State Authority shall issue a duplicate certificate marked as such within one day of the application.
