Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the dealer in medical devices shall apply to the National Medical Commission in Form 41 for a duplicate, accompanied by a fee of five hundred rupees, and the National Medical Commission shall issue a duplicate certificate marked as such within twenty-one days of the application.
