Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the dependant of a deceased worker shall apply to the Medical Benefit Council in Form 1 for a duplicate, accompanied by a fee of ten rupees, and the Medical Benefit Council shall issue a duplicate certificate marked as such within forty-two days of the application.
