Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the distribution of liquefied petroleum gas is carried on, the particulars of every licence held by the applicant and such other particulars as are specified in Form III, and shall be signed by the applicant or his authorised agent.
