Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurer shall apply to the Insurance Ombudsman in Form IRDA-3 for a duplicate, accompanied by a fee of one lakh rupees, and the Insurance Ombudsman shall issue a duplicate certificate marked as such within fifteen days of the application.
