Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the designated consumer shall apply to the Central Commission in Form V for a duplicate, accompanied by a fee of five thousand rupees, and the Central Commission shall issue a duplicate certificate marked as such within thirty days of the application.
