Surrender of certificate.—Every registered medical practitioner who ceases to carry on the practice of medicine shall, within forty-five days of such cessation, surrender the certificate of registration to the Licensing Authority in person or by registered post, and shall inform it in Form 21 of the manner in which every batch held by him has been disposed of.
