Application for registration.—Every insurance medical practitioner carrying on the provision of medical benefit shall make an application for registration to the Employees' Insurance Court in Form 2, within seven days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every medical certificate held by him on the date of the application, and shall keep a copy of the application and of the statement for thirty days from that date.
