Application for registration.—Every employer of a maternity beneficiary carrying on the employment of women entitled to maternity benefit shall make an application for registration to the Standing Committee in Form 12, within twenty-one days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every register held by him on the date of the application, and shall keep a copy of the application and of the statement for thirty days from that date.
