Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a vaccination centre shall apply to the Government of Odisha in Form 20-B for a duplicate, accompanied by a fee of two thousand rupees, and the Government of Odisha shall issue a duplicate certificate marked as such within thirty days of the application.
