Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the blood bank shall apply to the Appropriate Authority in Form 41 for a duplicate, accompanied by a fee of five hundred rupees, and the Appropriate Authority shall issue a duplicate certificate marked as such within seven days of the application.
