Intimation of change.—Every unorganised worker shall intimate to the Medical Benefit Council, in Form 16 and within fifteen days of the change, any change in the particulars furnished in the application for registration, and the Medical Benefit Council shall amend the certificate accordingly on payment of a fee of five hundred rupees.
