Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the institution shall apply to the Regulatory Authority in Form D for a duplicate, accompanied by a fee of one thousand rupees, and the Regulatory Authority shall issue a duplicate certificate marked as such within fifteen days of the application.
