Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurance intermediary shall apply to the Central Government in Form IRDA-5 for a duplicate, accompanied by a fee of five thousand rupees, and the Central Government shall issue a duplicate certificate marked as such within thirty days of the application.
