Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the enrolment of subscribers in a group is carried on, the particulars of every scheme held by the applicant and such other particulars as are specified in Form 11, and shall be signed by the applicant or his authorised agent.
