Surrender of certificate.—Every employee who ceases to carry on the receipt of wages in a covered establishment shall, within fifteen days of such cessation, surrender the certificate of registration to the Welfare Board in person or by registered post, and shall inform it in Form 5 of the manner in which every medical certificate held by him has been disposed of.
