Application for registration.—Every employee carrying on the receipt of wages in a covered establishment shall make an application for registration to the Welfare Board in Form 16, within sixty days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every medical certificate held by him on the date of the application, and shall keep a copy of the application and of the statement for sixty days from that date.
