Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the person carrying on a regulated activity shall apply to the Appellate Authority in Form A for a duplicate, accompanied by a fee of two thousand rupees, and the Appellate Authority shall issue a duplicate certificate marked as such within sixty days of the application.
