Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the plying of a tourist vehicle is carried on, the particulars of every conservation plan held by the applicant and such other particulars as are specified in Form VI, and shall be signed by the applicant or his authorised agent.
