Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the carrying on of business or profession is carried on, the particulars of every certificate held by the applicant and such other particulars as are specified in Form 26Q, and shall be signed by the applicant or his authorised agent.
