Intimation of change.—Every diagnostic laboratory shall intimate to the Director of Health Services, in Form 20 and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the Director of Health Services shall amend the certificate accordingly on payment of a fee of five hundred rupees.
