Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a critical facility shall apply to the District Authority in Form D for a duplicate, accompanied by a fee of five thousand rupees, and the District Authority shall issue a duplicate certificate marked as such within fifteen days of the application.
