Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the agent for collection of contributions shall apply to the Medical Benefit Council in Form 6 for a duplicate, accompanied by a fee of fifty rupees, and the Medical Benefit Council shall issue a duplicate certificate marked as such within ninety days of the application.
