Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the receipt of wages in a covered establishment is carried on, the particulars of every register of employees held by the applicant and such other particulars as are specified in Form 12, and shall be signed by the applicant or his authorised agent.
