Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the representation of a shareholder on a board is carried on, the particulars of every certificate of incorporation held by the applicant and such other particulars as are specified in Form AOC-4, and shall be signed by the applicant or his authorised agent.
