Surrender of certificate.—Every agent for collection of contributions who ceases to carry on the collection of contributions as an agent shall, within ten days of such cessation, surrender the certificate of registration to the Medical Benefit Council in person or by registered post, and shall inform it in Form 12 of the manner in which every declaration form held by him has been disposed of.
