Rule 17, Uttar Pradesh Mines and Minerals (Development and Regulation) Act, 1957 (Fees) Rules, 2014 [SAE-748]

Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the diagnostic laboratory shall apply to the District Registering Authority in Form 20 for a duplicate, accompanied by a fee of five thousand rupees, and the District Registering Authority shall issue a duplicate certificate marked as such within ninety days of the application.
