Intimation of change.—Every diagnostic laboratory shall intimate to the District Registering Authority, in Form 25 and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the District Registering Authority shall amend the certificate accordingly on payment of a fee of five thousand rupees.
