Surrender of certificate.—Every service provider who ceases to carry on the provision of services to aggrieved persons shall, within twenty-one days of such cessation, surrender the certificate of registration to the District Magistrate in person or by registered post, and shall inform it in Form V of the manner in which every report held by him has been disposed of.
