Surrender of certificate.—Every operator of a vaccination centre who ceases to carry on the administration of vaccines shall, within fourteen days of such cessation, surrender the certificate of registration to the Director of Health Services in person or by registered post, and shall inform it in Form 25 of the manner in which every test report held by him has been disposed of.
