Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the certifying medical authority shall apply to the National Trust in Form V for a duplicate, accompanied by a fee of two hundred rupees, and the National Trust shall issue a duplicate certificate marked as such within ninety days of the application.
