Application for registration.—Every certifying medical authority carrying on the certification of disability shall make an application for registration to the National Trust in Form IV, within sixty days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every register held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
