Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurance marketing firm shall apply to the Insurance Information Bureau in Form IRDA-6 for a duplicate, accompanied by a fee of five thousand rupees, and the Insurance Information Bureau shall issue a duplicate certificate marked as such within twenty-one days of the application.
