Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the actuary shall apply to the Tariff Advisory Committee in Form IRDA-5 for a duplicate, accompanied by a fee of ten thousand rupees, and the Tariff Advisory Committee shall issue a duplicate certificate marked as such within forty-five days of the application.
