Application for registration.—Every policy-holder carrying on the holding of a policy shall make an application for registration to the Insurance Information Bureau in Form IRDA-3, within ninety days of the commencement of these rules, accompanied by a fee of fifty thousand rupees and a statement of every cover note held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
